I've been using cyberonic dsl for close to three years.  speed is so-so, 
has a static ip, no problems with nat, no closed ports - $50 a month. 
Tech support is nice because you tell them you ran a traceroute and they 
know what that means, and they don't tell you to reboot windows.  I run 
a web server, mail server, etc.  never any problems.  One major outage 
of over 4 hours in the past 3 years.  Did have some minor issues when 
they converted their network which prompted a router upgrade, but other 
than that, they've been great.  I think you can get 5 static ips for $5 
more a month, and higher speed for another $5 a month, but it's been a 
while since i've checked their pricing.
